To receive an Individual Settlement Payment as part of the Settlement in Ford v. Genesis Financial Solutions, Inc., you must submit a Claim Form either in paper using the attached form or complete the claim form electronically on the Settlement Website, www.MarylandGenesisSettlement.com by June 29, 2026.
Your claim will be considered only if you comply with all of the following conditions:
- You must be a person to whom a Class Notice was addressed or you must be listed as a Settlement Class Member on the Ford v. Genesis Financial Solutions, Inc. Class List. If you have a question about whether you are a Settlement Class Member eligible to submit a claim, please contact the Settlement Administrator at 1-866-274-4004.
- You must accurately complete all required portions of the Claim Form by June 29, 2026.
- You have two options to complete a Claim Form:
(1) ELECTRONICALLY SUBMIT the Claim Form using the Settlement Administrator’s Settlement Website, located at www.MarylandGenesisSettlement.com. When you successfully complete the online Claim Form, you will receive a receipt that your claim has been submitted.
Or
(2) MAIL the completed Claim Form by First-Class U.S. Mail, postage prepaid, postmarked no later than June 29, 2026, to:

- If you do not complete and electronically submit or mail the Claim Form by June 29, 2026, you cannot receive any payment from the Settlement. So that you will have a record of the date of your mailing of the Claim Form and its receipt by the Settlement Administrator, you are advised to use (but are not required to use) either the Settlement Website or U.S. Mail by Certified Mail, Return Receipt Requested.
